Lameness in Growing Dogs: When Further Investigation Is Needed?

Lameness in a young dog can understandably cause concern for owners. While some cases may result from a minor strain or soft-tissue injury and improve with appropriate rest, persistent or recurrent lameness can indicate an underlying orthopaedic problem requiring further investigation.
For veterinary professionals, deciding when to move from clinical examination and monitoring to diagnostic imaging is an important part of managing these cases.
Understanding lameness in young dogs
Lameness is a clinical sign rather than a diagnosis. It may result from pain, weakness or a mechanical problem affecting a limb.
In young and growing dogs, potential causes range from minor injuries to fractures and developmental orthopaedic conditions. A dog’s age, breed, activity level and clinical history can all help guide the investigation.
A thorough orthopaedic examination is therefore essential. This allows the veterinary surgeon to assess the dog’s gait, identify areas of pain or swelling, evaluate joint movement and determine where the problem is most likely to originate.
Owners can also provide valuable information. Knowing when the lameness began, whether it appeared suddenly or gradually, and whether it changes following exercise or rest can help build a clearer clinical picture.
Does every lame young dog need imaging?
Not necessarily!
Where lameness is mild and recent, with no indication of significant trauma or concerning findings during examination, a veterinary surgeon may initially recommend conservative management. This can involve restricting activity and monitoring the dog’s progress.
However, monitoring should have a clear purpose and timeframe. If the dog does not improve as expected, the lameness returns or symptoms worsen, reassessment is important.
Persistent lameness in a growing dog should not simply be dismissed as “growing pains”, as clinically significant orthopaedic conditions can occur in young animals.
When should diagnostic imaging be considered?
Imaging may be appropriate when lameness is persistent, recurrent or worsening, or where examination identifies significant pain, swelling, joint instability or reduced range of movement.
It may also be recommended when trauma is suspected, a developmental condition is being considered or the physical examination does not provide a clear explanation for the dog’s symptoms.
The decision is individual to each patient. Age, breed, skeletal maturity, severity of symptoms and examination findings can all influence whether imaging is required.
Radiography and advanced imaging
Radiography is commonly the first imaging technique used to investigate orthopaedic problems. X-rays can provide valuable information about bones and joints and may identify fractures, developmental abnormalities and changes affecting joint surfaces.
Good positioning and appropriate radiographic views are essential for obtaining useful diagnostic images. Sedation or anaesthesia may sometimes be recommended to ensure accurate positioning while keeping the patient comfortable.
However, radiographs have limitations. Some soft-tissue injuries and subtle abnormalities may not be clearly visible on an X-ray. A normal radiograph therefore does not always exclude an underlying problem.
Where clinical signs continue despite inconclusive radiographs, advanced diagnostic imaging may be considered. Depending on the suspected condition, this could include CT, MRI or ultrasound. Each technique offers different advantages, so imaging should ideally be selected according to the specific clinical question being investigated.
Reassessment is an important part of diagnosis
Lameness does not always reveal its cause during the first consultation. Clinical signs may initially be subtle, or the dog’s condition may change over time.
Planned reassessment is therefore particularly important when conservative management is chosen. Owners should monitor changes in movement, willingness to exercise and everyday behaviours such as climbing stairs or jumping.
Ultimately, diagnostic imaging should form part of a structured approach rather than being viewed as an automatic response to lameness. Combining a detailed history, careful orthopaedic examination, appropriate monitoring and targeted imaging can help identify the underlying problem and support the best possible outcome for a young dog.
