About the Role:
We are seeking a highly motivated and experienced Clinical Pharmacist to join a progressive and supportive GP group in Aylesbury. This is a full-time, patient-facing position across three well-integrated GP sites, offering the chance to work in a collaborative primary care setting.
You will play a key role in delivering high-quality pharmaceutical care, optimising medicines use, and supporting patient safety and wellbeing. Key Responsibilities:
Provide clinical pharmacy services, including medication reviews, medicines optimisation, and reconciliation
Work collaboratively with GPs, nurses, and other healthcare professionals
Support the management of long-term conditions and contribute to QOF and IIF targets
Deliver patient education and counselling on safe and effective use of medicines
Ensure compliance with relevant legislation, clinical guidelines, and best practice
Assist in the development and implementation of prescribing policies and clinical protocols
Registered Pharmacist with the General Pharmaceutical Council (GPhC)
1–3 years’ experience in a Primary Care or General Practice setting (essential)
Postgraduate qualification in pharmacy or clinical pharmacy (essential)
Strong clinical knowledge and a patient-centred approach
Excellent communication and interpersonal skills
Ability to work both independently and as part of a multidisciplinary team
Salary up to £50,000 per annum, depending on experience
Full-time role (Monday to Friday) with flexible working options
